- Vital signs are a rapid, objective method of assessing a patient's clinical status and well-being. There are five vital signs: blood pressure, heart (pulse) rate, respiratory (breathing) rate, temperature, and as of 2001, pain score. [Source: Encyclopedia of Health Care Management; Vital Signs]
