  • Case management is a clinical tool that is used to increase the efficiency and effectiveness of client health and social services and control costs at the same time. The Case Management Society of America (CMSA) defines case management as a collaborative approach to assess, plan, facilitate, and advocate for services that meet the health needs of individuals and to increase the quality and cost-effectiveness of client outcomes.
